Original Academic Certificates, original JKT/JKU certificate, original birth certificate, NIDA card or National Identification number from NIDA, Copies of NIDA cards or NIDA National Identification numbers of parents and close people on the father’s and mother’s sides (such as aunts, uncles, grandfathers, grandmothers, etc.), passport size 06 (blue background), and 5 copies for each certificate specified above.
